Responsibilities

- Assisting various departments within DHSD when complex client issues arise or issues outside regular processes create the need to problem solve in order to navigate health care systems.
- Collaborating with patients, their families and medical professionals to complete assessments and coordinate a plan of care.
- Facilitate multiple care aspects and setting up case conferences between patients, families, physicians and other health professionals
- Develop effective working relationships with various departments within Labrador Grenfell Health and other health regions throughout the entire case management process
- Collecting and providing information in regards to financial assistance and community programs that may be of help to patients once discharged.
- Navigation of medical transportation system for income support clients
- Networking with various external agencies to organize supportive services according to the needs of patients.
- Coordinate visits between family members and individuals placed in long-term care, personal care homes and family caregiver homes
- Coordinating interpreter/translator services for case conferences as required
- Assist individuals who have concerns/complaints about health care systems and help to resolve/address issues.
- Assist individuals with finding appropriate housing options
- Other related duties as required
Qualifications
- Bachelor’s Degree in Social Work with a minimum 5-10 years case management experience – preferably a combination of experience in a health care setting (inpatient, emergency, outpatient, mental health, community supports, etc…) and community.
- Registered with Newfoundland and Labrador College of Social Workers
- Demonstrated ability to problem solve, reach out to key agencies/supports for clients, advocate on behalf of clients and families
- Proven ability to work effectively with Indigenous populations
- Strong assessment skills, ability to build rapport/ engage clients, build a trusting relationship and develop individual case plans
- Knowledge of group processes (consensus building, conflict resolution, case management, etc…)
- Good organizational skills
- Demonstrated effective written and interpersonal communication skills.
- Ability to multitask and work flexible hours
- Ability to work independently and as part of a team
Assets
- Bilingual in Inuktitut and English
- Knowledge of the Labrador Inuit culture and northern communities
