Responsibilities
View English and Inuktitut version
- Coordinates case management with client, family and care providers
- Collaborates with the health care team on all aspects of service delivery
- Receives and sends referrals for clients with varying health needs
- Promotes self-care and independence by providing instruction and education to individuals, families, and other groups as necessary
- Maintains client records and statistical information and prepares reports as required
- Assists in the orientation of new staff
- Acts as a preceptor and actively participates in the coordination of education programs for nursing students as required
- Provides an advocacy role for clients to community and institution based service
- Carries out monthly and quarterly reporting as required
- Report incidents to direct supervisor (eg. Adverse Event Following Immunization)
- Travel as required
- Work flexible hours as required
Qualifications
- Graduation from an approved School of Nursing with a Bachelor degree in Nursing
- One year of Community Health Nursing experience is preferred
- Knowledge in Primary Health Care and community development
- Ability to be self-directed and exercise independent nursing judgement in a community based environment with limited supervision, work collaboratively with team members and the community
- Ability to assess community needs and assets and to develop, implement and evaluate community health nursing programs and healthy public policy in a population health framework
- Currently registered with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) or able to obtain registration
- Current CPR certification

- All travel and accommodations to and from communities will be provided at defined locations within the province of Newfoundland and Labrador, at no cost.
