Responsibilities

  • Coordinates case management with client, family and care providers
  • Collaborates with the health care team on all aspects of service delivery
  • Receives and sends referrals for clients with varying health needs
  • Promotes self-care and independence by providing instruction and education to individuals, families, and other groups as necessary
  • Maintains client records and statistical information and prepares reports as required
  • Assists in the orientation of new staff
  • Acts as a preceptor and actively participates in the coordination of education programs for nursing students as required
  • Provides an advocacy role for clients to community and institution based service
  • Carries out monthly and quarterly reporting as required
  • Report incidents to direct supervisor (eg. Adverse Event Following Immunization)
  • Travel as required
  • Work flexible hours as required
Qualifications
  • Graduation from an approved School of Nursing with a Bachelor degree in Nursing
  • One year of Community Health Nursing experience is preferred
  • Knowledge in Primary Health Care and community development
  • Ability to be self-directed and exercise independent nursing judgement in a community based environment with limited supervision, work collaboratively with team members and the community
  • Ability to assess community needs and assets and to develop, implement and evaluate community health nursing programs and healthy public policy in a population health framework
  • Currently registered with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) or able to obtain registration
  • Current CPR certification
Assets
  • Bilingual in Inuktitut and English

 

Travel & Accommodations
  • All travel and accommodations to and from communities will be provided at defined locations within the province of Newfoundland and Labrador, at no cost.
