Responsibilities

- Assist various departments within the Department of Health and Social Development (DHSD) when mental health client issues arise or issues outside regular processes create the need to problem solve in order to navigate mental health and addictions services and related systems;
- Collaborate with clients, their families and other pertinent professionals to assess the need for services, co-coordinate plans of care for those requiring mental health services, intervention or community reintegration support plans following hospital admissions;
- Facilitate multiple care aspects, such as, but not limited to: case coordination, and information sharing. Coordinating case conferences between patients, families, physicians, psychiatrists and other health professionals who are determined to be within the circle of care;
- Develop effective working relationships with various departments such as, but not limited to: the mental health unit, psychiatric services, emergency, inpatient departments within LHC and other health regions throughout the entire case coordination process;
- Provide information and service connections to services such as, financial assistance, housing options, community reintegration planning, to ensure best service outcomes, while considering equitable access to services that support the social determinants of Inuit Health;
- Coordinate interpreter/translator services for case conferences as required;
- Assist beneficiaries of the Labrador Inuit Land Claims Agreement (LILCA) in accessing private mental health services, as required with consideration to NIHB guidelines, and at times using a case management approach;
- Maintain up to date information related to mental health NIHB program coverage and implementing a communications system to keep both internal and external service providers up to date;
- Lead the development and continuous review and updating of the NG, DHSD NIHB Mental Health Beneficiary Guidebook;
- Lead the process, implementation and ongoing evaluation of the ADS interface for real-time processing of mental health NIHB related travel in collaboration with Labrador-Grenfell Zone and other partners (as required);
- Reviewing and making approval decisions when called upon by the Mental Health Analyst, in relation to NIHB mental health program area
- Other duties as required
Qualifications
- Meet one of the following requirements:
- Bachelor’s Degree in Social Work (Master’s Preferred) and eligibility for Registration with the Newfoundland Labrador Association of Social Workers with a minimum 5 years case management and service navigation experience – preferably in a mental health setting
- Bachelor of Nursing Degree and eligibility for Registration with the College of Registered Nurses of Newfoundland and Labrador with a minimum 5 years of related experience.
- Strong knowledge of the provincial Mental Health Care and Treatment Act (MHCTA) and ability to advocate while considering aspects of the Act
- Proven ability to work effectively with Indigenous populations – understanding the impacts of colonization, historical trauma in relation to systems barriers and social determinants of Inuit health `
- Strong assessment skills, ability to build rapport/ engage clients, build a trusting relationship and develop individual case plans
- Ability to work effectively with key partners within various systems (physicians, MH staff, community supports, nursing staff and other health care professionals) to provide effective case management services for clients
- Ability to demonstrate personal use of stress reduction, time management and healthy living techniques
- Demonstrated effective written and interpersonal communication skills.
- Ability to multitask and work flexible hours and maintain good organizational skills
- Ability to work independently and as part of a team.
- Life-long learner, creative, willing to explore innovative ideas and seek out best/promising practices
- Use of a personal vehicle is required
