Responsibilities

- Responsible for case management of individuals with multiple and complex needs and/or at risk of homelessness in the Upper Lake Melville area.
- Using an Intensive Case Management and Housing First Approach, provide assessment, counselling and intervention, as well as support in life skill development to address the multiple and complex bio/psycho/social/functional needs of individuals needing supportive housing
- Coordinate government and community services, e.g. Labrador Grenfell Health (Mental Health Team – including Psychiatrist, Home Care and medical staff) and Justice (Labrador Correctional Centre, Probation, Parole, etc.) and other agencies and groups as part of a multidisciplinary team
- Meet as required with supported individuals and establish and monitor individual support plans to address any challenges and encourage continual progress in all life areas
- Provide outreach and mobile crisis intervention
- Continuously explore best practices/creative solutions to housing and supportive services for individuals with multiple challenges.
Qualifications
- Minimum Bachelor’s Degree in Social Work (Master’s preferred) or other behavioural science with current registration with NLASW.
- Minimum 2 years case management and counselling experience.
- On call and personal use of vehicle will be required.
